These are just some of the transition requirements under IDEA 2004.Transition planning · is used to identify and develop goals which · need to be accomplished during the current school year. · assist the student in meeting his post-high school goals.A transition plan· In Kansas must begin at age 14 or younger, if necessary.· Is part of the Individualized Education Program (IEP) that outlines transition goals and services for the student. · Is based on a high school student’s individual · needs · strengths · skills· interests· Be focused on improving the student’s academic and functional achievement , including· Postsecondary Education· Vocational Education· Integrated employment· Adult Services · Community Participation· Requires that a statement of the student’s transition goals and services be part of the plan.· Must include at least one postsecondary goal for education or training, employment and independent living, if appropriate.· Must include the courses of study.· Meet the requirements of Indicator 13.Join Us and Learn more about Individualized Transition Planning .
